To help us get everything ready, we kindly ask that all families complete & submit their enrollment paperwork by Friday, July 24.
The following forms & paperwork must be submitted for each enrolled child:
- Operational Policy and Agreement Form (submit signature page)
- Hearing and Vision Screening (for children 4 & 5 years as of Sept. 1)
- Most recent immunization record or completed Affidavit for delays or exemption. (Please note that we are able to notarize your completed Affidavit forms.)
The following form must be completed for any enrolled child with a known allergy:
- Allergy Action Plan & FARE Form (physician’s signature required)
The following form must be completed for any newly enrolled child:
- Health Statement
- Please be on the lookout for a digital Tuition Express form that will be sent via email to all new families on July 7. For credit card and ACH requirements please click here.
Completed forms can be submitted in one of the two following ways:
- Digitally: Email digital or scanned document to CDC@theheights.org. (Please do not submit pictures of completed enrollment forms. We need a clear copy to hold on file for Texas State Licensing)
- Paper copy: Drop off in the secure drop box located under the Family Entrance Doorbell (accessible 24/7)
