Below you will find the required enrollment forms for the 2024-2025 school year. Please complete the appropriate forms for each of your enrolled children. Enrollment forms can be completed digitally or on hardcopy. Digital forms can be submitted via email, CDC@theheights.org. Hard copies can be drop off at the CDC desk during Summer Camps or placed in the secure drop box at the Family Entrance. All enrollment forms must be received by August 1, 2024.
