25-26 Enrollment Forms

To help us get everything ready, we kindly ask that all families complete & submit their enrollment paperwork by Friday, July 25.




The following forms & paperwork must be submitted for each enrolled child:

The following form must be completed for any enrolled child with a known allergy:

Completed forms can be submitted and one of the two following ways:

  • Digitally: Email to CDC@theheights.org
  • Paper copy: Drop off in the secure drop box located under the Family Entrance Doorbell (accessible 24/7)
