To help us get everything ready, we kindly ask that all families complete & submit their enrollment paperwork by Friday, July 25.
The following forms & paperwork must be submitted for each enrolled child:
- Operational Policy and Agreement Form (submit signature page)
- Tuition Express Form (updated yearly)
- Hearing and Vision Screening (for children 4 & 5 years as of Sept. 1)
The following form must be completed for any enrolled child with a known allergy:
- Allergy Action Plan & FARE Form (physician’s signature required)
The following form must be completed for any newly enrolled child:
Completed forms can be submitted and one of the two following ways:
- Digitally: Email to CDC@theheights.org
- Paper copy: Drop off in the secure drop box located under the Family Entrance Doorbell (accessible 24/7)
